As communicated in December 2009, Alliance Claims Solutions has recently joined SafeliteŽ Solutions. Beginning February 19th, 2010, and continuing over the next 3 months, Alliance will be converting all dispatches from an EIGHT digit to a SIX digit referral number. In order to ensure a smooth transition, you will need to perform the following steps:

1. Establish TWO accounts for Alliance Claims Solutions in your Point of Sale or invoicing system, effective February 19th, 2010.
   1. One account will be for all EIGHT digit referral numbers and should be billed WITH SALES TAX to the following address:

      Cedar Rapids, IA 52406

   2. Second account will be for all SIX digit referral numbers and should be billed WITHOUT SALES TAX to the following address:

      Alliance Claims Solutions
      DBA: Safelite Solutions
      P.O Box 18277
      Columbus, OH 43218-2177
      For all SIX digit referral numbers, you can also send your invoice via EDI using mailbox SAFL 107, and program I.D. 00408. If you do not have EDI capability, you may use SGC Network.com to send invoices electronically FREE of charge, which will help facilitate the timely payment of your invoice.
      In addition, our Multijurisdiction Tax Exemption Certificate is attached for all SIX digit referral numbers.

2. Please follow the billing instructions on either the EIGHT digit or SIX digit work orders. Note that many of our clients require prior approval and proof of purchase documentation in certain instances. These items generally include non-NAGS published parts, OEM parts, tinting, and other miscellaneous charges. Any additional parts, labor or change in the work to be performed must be authorized prior to doing the work. Please contact the appropriate email or phone number on each work order.
